Facilities Use Agreement

Use of the department's facilities requires that you agree to certain conditions (Agreement 1.02 as updated on August 7, 2008.)

Personal Oversight Required. I agree to manage the equipment within the facilities responsibly and to keep it constantly under my own personal care. In particular,

• I agree not to loan, rent, or otherwise allow any other individual, entity, or agency to use the facilities or associated equipment.
• I agree only allow equipment within the facilities to be operated by persons who have been trained and certified in its specific use by the Department Communication or UW Educational Outreach.

No Modifications. I agree not to modify the computers and other equipment located in the department's facilities. In particular,

• I agree not to install software
• I agree not to attach personal devices to computers or other gear
• I agree not to attach personal devices to the phone or computer networks

Mandatory Reporting. I agree that I will use all equipment and media with due care and that I will promptly report any operational or other problems to Nika Pelc (pelcdv@u).

Defaults Restored. I agree that, after each use, I will return all equipment to its proper default conditions (e.g., video monitors turned off), that I will restore furniture and other items to their sanctioned locations, and that I will appropriately remove my papers, books, food, garbage, and other items that are associated with my use.

No Storage. I agree not to store personal items within departmental facilities except as explicitly authorized.

No Consumption. I agree not to eat in proximity to equipment or to have drinks in uncovered containers near any kind of gear.

Protect Physical Security. I agree not to allow unauthorized individuals (e.g., persons without their own personal door codes) into the labs and to immediately report such persons to the University police. I agree to help assure that doors are closed and locked.

No Shared Codes or Keys. I agree not to share, duplicate, distribute or to otherwise allow others to know my keys or private access codes (e.g., door code, UWNetID, etc). I agree to make every possible effort to safeguard the security and secrecy of my keys and access codes.

No Animals. Except for seeing-eye dogs engaged in navigation tasks, I agree not to bring animals into the labs.

No UW Liability. I agree that the University of Washington, the Department of Communication, and their employees, agencies, affiliates, and related personnel shall not be liable for any loss or damage that results in any way from use of the equipment or facilities. Additionally, I agree that the University, the Department, and their employees, agencies, and affiliates will be held harmless from any claims, actions, suits, proceedings, costs, expenses, damages and liabilities, including attorney's fees arising from, connected with, or resulting from, any use of the facilities and equipment or from any and all for any acts or omissions of associated personnel.

Public Acknowledgment Required. I agree to publicly acknowledge the Department of Commnication within every production, publication, and other public display that directly or indirectly benefits from the use of provided facilities and that the Department must approve such acknowledgement in writing prior to any form of public release.

Copy Required. I agree to provide the Department of Communication with a no-cost copy of my productions, publications, and other public displays that directly or indirectly benefit from the use of provided facilities, if requested.

Monitoring Authorized. I agree to allow the University of Washington and Department of Communication to monitor my activities electronically (including, but not limited to, taking and storing video and images of me).

I understand that failure to comply with this Facilities Agreement may result in my being denied access to resources and in having other actions possibly taken against me.

I understand that legal action may be taken against me for failure to return keys when requested and that University records (e.g., transcripts, degrees) may be withheld if keys are returned when due.

I acknowledge that I have read and understood all of these conditions and that I agree to be bound by these terms.
